Web Resources

The LHS teachers' web library contains links to valuable sites for teachers of all subject areas. Originally compiled by Jeff Baxter, this site has been added to by many teachers. The students' web library also contains links to excellent sources of information.

Forms

District forms are available at the district's intranet site (accessible from school computers).

 

Syllabus template: Use this to format your course syllabus.

Quarterly Objectives template: Use this form to document which state standards are addressed each quarter in a course.

School Improvement Audit Sheet: Used to document reading, writing, and math strategy use by December 1 and May 1. Document is in Word format.

SSR Reading Log: Students record their reading progress on this form; should be printed two-sided.
Appropriate Internet Sites: Used to request a change in the filter system, to add or delete a site.
USD 453 Requisition in Excel format. Teachers should photocopy the mission onto the back of the form to make it official. This form will compute the shipping and handling at 10%; if the shipping cost is different, simply override the default by entering the amount. This version of the requisition is in Excel format and can be saved with your account number, etc., in your network space, so that you don
't start from scratch each time you write a requisition.
