Health Services
A nurse is assigned to LHS on a daily basis while students are in school. If the nurse is absent, an on-call nurse is available to handle emergencies.
The nurse provides first aid services and carries out doctors' orders regarding medications and treatment. Students obtain a "referral to nurse" slip in order to make a visit to the nurse's office.
A completed and signed Health Care Assessment form (see forms below) alerts the nurse, and through her the teachers, of any health problems that could affect a student's safety or ability to succeed in the classroom. It also provides permission to obtain emergency medical/dental care in the event a parent cannot be contacted.
Vision, Hearing and Blood Pressure Screening
In accordance with state guidelines, vision, hearing, and blood pressure screening tests are performed each year on each 10th and 12th grade student, any new student, and any student who had a problem in the previous year. Referrals are sent to parents only if a problem is found.
Immunizations
State law requires that an immunization record be on file at the school for each student. The nurse reviews the records and notifies parents if an updated shot is needed. Students must have up-to-date immunizations in order to stay in school.
Medications
School Board Policy states that students may not carry any medication except asthma inhalers and/or Epipens. Parents need to provide any over-the-counter medication (i.e., aspirin, Tylenol, Midol, etc.) in its original bottle and a signed Permission for Dispensing form. The bottle will be labeled with the student's name and locked in the nurse's office. Prescription medications must be in the original bottle with written doctor's orders.
Forms
Forms for School Health Services must have an original parent/guardian signature. They can be filled out online but cannot be submitted electronically. Print, sign, and turn the form in to the school nurse.
Health Assessment: This form must be completed and signed for every LHS student.
Permission for Dispensing of Medication: This form is needed for the school nurse to dispense medication (see Medications above).
